Your Appointment and Medical Records

For your first appointment, we ask you to arrange for us to have your medical records from your other physicians that may have treated you for your pain. It is important that we have information on the history of your condition, prior treatments, and tests you have had performed. Please bring your insurance information.

The initial consultation allows you ample time with a physician to discuss your pain; its initial onset; the degree, severity, and frequency, and the various treatments you may have already tried. Then, we create a plan that fits your condition. This might include specialized injection therapy using fluoroscopy, physical therapy, diet, exercise, and/or medication. Your treatment will be fully explained to you and your questions answered prior to commencing with any course of action.

If you are scheduled for a procedure in our office, we ask that you do not eat or drink anything 8 hours prior to the procedure. If you are a diabetic, be sure to discuss your eating and medication schedule with your physician.

You may need to stop taking certain medications several days before your procedure. Remind your doctor of all prescription and over-the-counter medications you take, including herbal and vitamin supplements. Your doctor will tell you if and when you need to discontinue your medications.

Inform your doctor if you develop a cold, fever, or flu symptoms before your scheduled procedure appointment.

If you are unable to keep your scheduled appointment, please contact us at least 24 hours in advance so that we may be able to offer that time to another patient.

Your portion of our fee, including co-pay or deductible, is due at the time of service. For your convenience, we will file a claim with your insurance company.
